Background: Some have expressed interest in having access to fillable C-SEP forms. One convenient option for providing these is through online "forms". We've decided to experiment with a few below. These work as follows: Select the form you wish to complete. Open it in your browser and complete it. Then, using your internet browser menu options, select "print". This should prompt your browser to convert the form into a PDF document. Be certain to save the form in pdf format before closing your browser window.

When using these forms, please understand: First, the E-Form will look better in your browser if you adjust the size of the window to roughly resemble the dimensions of a document. Second, a background code in the page better formats the form for printing, so regardless of how chaotic it may look in your browser, the pdf results should be more uniform and consistent. Third, any information you enter into the online form IS NOT SAVED! If you close your browser before converting it to PDF and saving that PDF, your data will be lost. Finally, and related, no records or history of the data entered are kept on our end. Any data you input into a form is only temporarily stored on your computer and browser. We DO NOT collect any information on the backend! Our server only hosts the html form as a webpage, it does not (temporarily or permanently) collect, save or process any data entered.
